We accept most major insurance plans!
CVI is in-network with most major insurance plans. If you have an in-network or out-of-network PPO plan, you can schedule an appointment without a referral. For your convenience, a list of our current in-network plans is included below. Our practice is constantly expanding our insurance carriers, so if you do not see your health plan on this list then please don’t hesitate to contact our office and ask if we work with yours!

Please note that we exclusively accept Covered California plans through HealthNet. We do not participate with other state-sponsored Covered California plans, such as BlueShield Covered California.
For HMO plans, a referral authorization is required. Please contact our office, and we will gladly provide the details you need to obtain a referral. While approved referrals for office visits are not common, we have seen greater success with HMO authorizations for our treatments.
