Prostate artery embolization or PAE is a challenging embolization but an impactful procedure in Interventional Radiology for enlarged prostate symptoms and/or hematuria, blood in urine.
⭐️ 𝗪𝐡y is PAE the most 𝐜𝐡𝐚𝐥𝐥𝐞𝐧𝐠𝐢𝐧𝐠 embolization? These arteries can be small with varying anatomy. Above demonstrates difficult bilateral origins from the obturator artery. There is also supply to the penis on the right. Regardless in appropriate hands it can be done well.
💡PAE is a relatively new non-surgical treatment option for an enlarged prostate or BPH. 𝗪𝐡𝐨 𝐠𝐞𝐭𝐬 𝐚𝐧 𝐞𝐧𝐥𝐚𝐫𝐠𝐞𝐝 𝐩𝐫𝐨𝐬𝐭𝐚𝐭𝐞? Anyone that has a prostate!

Successful outcome requires skill and experience in visualizing the thyroid in 3D while watching a 2D ultrasound image during the ablation.

Before and After IPSS score of a patient who had Prostate Artery Embolization, PAE. These scores help determine the severity of patients prostate symptoms, the higher the number the worse the situation.

💡Cysts (dark areas) contain fluid and these are first aspirated using a tiny needle. Once the fluid has been removed, an RFA probe is moved through the thyroid nodule “ablating” the tissues. 💡Using ultrasound guidance a tiny tube called a catheter is placed through the skin and the fluid (black) is removed. Then a measured amount of a sclerotherapy medication is administered into the residual cystic sac causing it to close 🥳

The exact reason why spermatoceles develop is unknown but it is believed to be due to a blockage in one or more of the tubes within the epididymis.
𝐓𝐫𝐞𝐚𝐭𝐦𝐞𝐧𝐭𝐬: This can be treated with surgery or a non-surgical procedure called aspiration and sclerotherapy.

These are tiny loops of soft metal, typically made of platinum. The purpose of a coil is to block the flow of blood 🩸
They come in different size loops, lengths, and strengths depending on the vessel being deployed in. These coils are specially packed in a tiny tube and come out of the catheter tip to allow for proper placement.
⚠️ Appropriate sizing is essential to make sure they have a nice fit in their new home, your blood vessel 🙃

Prostate artery embolization or PAE is a challenging but impactful procedure in Interventional Radiology for BPH and or hematuria, blood in urine.
⭐️ While this is a low risk procedure, the biggest risk is non-target embolization of beads into organs outside of the prostate. In the above case there is communication of the prostate artery with the penis. A good procedural evaluation can identify these communicating branches so that they can be closed off before the administration of beads.
💡PAE is a relatively new non-surgical treatment option for an enlarged prostate or BPH. Microparticles are placed into the prostate artery blocking the blood flow to the prostate gland causing it to shrink in size and improve symptoms.
💡In 2018 the UK-NIH concluded that PAE is safe and effective to provide to patients with BPH and is now covered by their universal healthcare. “The safety profile and quicker return to normal activities may be seen as highly beneficial by patients considering PAE as an alternative treatment to TURP … place of PAE in the care pathway is between that of drugs and surgery.”
💡The American Urology Association still considers PAE as experimental compared to their treatments. Regardless, there are multiple studies showing it’s efficacy and safety, the treatment is FDA approved and currently covered by some insurance companies.

RFA has been around for over a decade and initially used by Interventional Radiologist for the treatment of cancers, such as the liver and lung. Interventional Radiologist now use microwave and cryoablation (freezing) for cancerous tumors. RFA however is well suited for the treatment of thyroid and parathyroid lesions.
How does RFA work? A special probe is placed into the nodule using ultrasound guidance. This probe then heats up the tissue using a radiofrequency. The heated tissue then shrinks and dies over time similar to cancerous tumors treated with ablation in other organs.

Prostate artery embolization or PAE is a challenging but impactful procedure in Interventional Radiology for BPH symptoms and/or hematuria, blood in urine.
⭐️ While this is a low risk procedure, the biggest risk is non-target embolization of beads into organs outside of the prostate. In the above case there is communication of the prostate artery with the penis. A good procedural evaluation can identify these communicating branches so that they can be closed off before the administration of beads.

🚑 Patient who presented with shortness of breath found to have blood clots in the lungs and heart.
🩺 A catheter a size larger than a Sharpie pen is placed through a vessel in the groin, and advanced into the heart and lungs. Using x-ray guidance and an ultrasound probe in the esophagus the blood clots can be visualized for removal. Suction is then performed removing the blood clots shown above, restoring blood flow.
📚 Blood clots in the lungs can present with shortness of breath, chest pressure, collapse and death. Large blood clots that cause abnormal pumping of the heart is called 𝐦𝐚𝐬𝐬𝐢𝐯𝐞 𝐩𝐮𝐥𝐦𝐨𝐧𝐚𝐫𝐲 𝐞𝐦𝐛𝐨𝐥𝐢𝐬𝐦 (𝐏𝐄), which is fatal without treatment.

🚨Emergency Uterine Artery Embolization #UAE performed for life threatening fibroid bleeding following cesarean delivery.
Images show small fibroids inside the uterus and the uterine arteries prior to embolization. This procedure was performed through the wrist, requiring no major incision or stitches.
⭐️One risk of a fibroid uterus is postpartum hemorrhage. While the risk is small the consequences can be life threatening 😬 Patient above received multiple blood transfusions however due to continued bleeding this was treated with the UAE procedure blocking the blood flow using a non-permanent gelatin mixture. Patient stabilized and bleeding resolved 👏🏼

A good procedural evaluation can identify these communicating branches so that they can be closed off before the administration of beads.
💡PAE is a relatively new non-surgical treatment option for an enlarged prostate or BPH. Microparticles are placed into the prostate artery blocking the blood flow to the prostate gland causing it to shrink in size and improve symptoms.

Before and After IPSS scores of a patient who had Prostate Artery Embolization, PAE. These scores help determine the severity of patients prostate symptoms, the higher the number the worse the situation.

🚑 Patient who presented with shortness of breath found to have blood clots in the lungs and heart.
🩺 A catheter a size larger than a Sharpie pen is placed through a vessel in the groin, and advanced into the heart and lungs. Using x-ray guidance and an ultrasound probe in the esophagus the blood clots can be visualized for removal. Suction is then performed removing the blood clots shown above, restoring blood flow.
📚 Blood clots in the lungs can present with shortness of breath, chest pressure, collapse and death. Large blood clots that cause abnormal pumping of the heart is called 𝐦𝐚𝐬𝐬𝐢𝐯𝐞 𝐩𝐮𝐥𝐦𝐨𝐧𝐚𝐫𝐲 𝐞𝐦𝐛𝐨𝐥𝐢𝐬𝐦 (𝐏𝐄), which is fatal without treatment.

Rare presentation of an invasive breast cancer to the axilla presenting with uncontrolled active bleeding.
Via the wrist, a catheter was advanced into the arm and contrast was injected allowing visualization of the supplying arteries. The blood flow was then cut off (embolization) using gelfoam and coils.
